Moonee Valley Care has implemented a reintegration program that incorporates evidence-based practices for individuals with mental health challenges transitioning out of incarceration. Our program is designed to work with participants' allied health professionals and includes mental health assessments and treatment, case management, housing assistance, employment and education support, community integration, peer support, and crisis management support. Our qualified support workers work closely with allied health professionals to ensure that participants receive the appropriate care and support. We have found that these steps have been effective in supporting successful transitions back into the community and reducing the risk of reoffending for our participants.
